  3. BPPV (Benign Paroxysmal Positional Vertigo) Assessment & Treatment: $185 (in-person only)
  4. Montreal Cognitive Assessment (MoCA): $185 (in-person only)

Lab Services:

  • Lab test ordering by your provider is included in your membership. However, you are responsible for paying any lab fees directly to the lab. You may be able to use your HSA or FSA to cover lab fees—please check with your insurance provider for eligibility.
